Some participants experience complex circumstances or significant barriers to putting supports in place. When Specialist Support Coordination is included in your NDIS plan, we work with you and the people involved to find practical ways forward.
Depending on your circumstances, we may collaborate with your family or informal supports, providers, allied health professionals, health services, housing services, government agencies and community organisations.
We help identify barriers, improve communication, explore appropriate options and coordinate a support network that better reflects your needs and goals. Your choice and control remain central.
