When you call or email us, we use the information you provide to respond to your enquiry or referral. Please avoid sending medical records, full NDIS plans or other sensitive documents by ordinary email unless our team has discussed an appropriate way to share them.
We will discuss consent before sharing participant information with other people or services. A referral enquiry is not blanket consent to share information with providers indefinitely. Contact us if you have questions about your information or would like to request access or correction.
This review site does not collect referral form entries. Before the WordPress site goes live, the final privacy policy must be checked against the selected forms, analytics, hosting, storage, retention and complaints procedures.
