Think about your goals, preferred communication style, location and the practical support you need. Ask providers about availability, service agreements and how they respond when something is not working. You can compare options before deciding.

Preparing for a plan review

Keep notes about what has worked, what has changed and where support gaps remain. Gather relevant reports and examples of progress, and consider the goals you want to discuss at your review.
